Access Social and Community

MAP provides disability support services to people in their homes, the community and socially throughout the Barossa, Gawler and northern suburbs. Disability support might include personal care, domestic assistance, appointments, managing the household and attending social engagements.

You choose how you want to engage and interact, and MAP finds the right people to support you on that journey.

To support clients in reaching their NDIS Goals, we match our experienced and skilled team members to ensure compatibility and suitability. Support can be provided in the home, out in the community or at social events and activities. We are guided by the clients wishes and are dedicated to ensuring our clients feel connected with their community and live their best lives.

Supported Independent Living (SIL)

MAP offers Supported Independent Living options for people with disability that require housing and support full-time in your home, or one of ours located in the Barossa. MAP will work with you to create a vibrant, secure, comfortable environment that reflects your personality.

At My Ability Pathway we are committed to ensuring that everyone has the opportunity to live a fulfilled and enriched life in a home that is not only comfortable but also suited to their unique needs. We recognise that for people with disability, achieving independence often requires specialised equipment and advanced technology. Our mission is to provide the necessary support to help you attain the level of independence you choose and deserve.

To achieve this, we carefully develop a personalised roster of support tailored to your inpidual goals and requirements under the NDIS. A comprehensive support plan is designed to assist with all aspects of personal care, health and wellbeing, access to community, family and social engagement, and household management, including financial and domestic needs. Whether it’s budgeting, managing household expenses, connecting with family or maintaining a clean and organised living space, our team is here to help every step of the way.

Our SIL services provide 24/7 support, including active or passive overnight care, to ensure your needs are met at any time of day or night. The support extends beyond the home keeping you connected to family, friends and helping you access the community, whether it’s for essential activities like shopping, attending medical appointments, or participating in social engagements. We understand the importance of staying connected with family, friends, and the broader community, which is why we provide assistance with attending events, engaging in sports activities, and participating in social gatherings.

Our Supported Independent Living (SIL) houses and dedicated team members are always available to ensure that you live your best and most comfortable life. At My Ability Pathway, we believe that with the right support, everyone can achieve their goals and enjoy a fulfilling, independent life.

Medium Term Accommodation (MTA)

Medium Term Accommodation is an option for people with a disability to secure appropriate housing for a shorter period of time, whilst explore a more permanent option. MAP works closely with individuals to secure MTA that enables them to live safely and securely.

An MTA is a good solution for inpiduals who are unable to remain in their current residence and who are actively seeking permanent appropriate housing solution. Our team members can support you in a new environment whilst you are exploring other long-term options.

Short Term Respite

MAP offers short term respite in the stunning Barossa Valley. Our short term respite service provides the comforts of quality respite in a private home, supported by experienced, caring team members.

MAP can also provide alternative options for short term respite at other locations around the state. A change of environment might be all you need for you and your informal supports to relax and recharge.

The MAP team will work with you to plan your stay including, where and how long you will stay, what support or equipment you may need during your stay and what activities you would like to be involved in. The Barossa Valley offers an abundance of activities including Farmers Market, Chocolate Factory, Nature Walks, Mini Golf, Bowling, Restaurants, Live Music, Breweries, Shopping and Wine Tasting. You can choose to relax and unwind or immerse yourself in the local communities and endless adventures available.
