Support Coordination

MAP Support Coordination services are delivered across South Australia by a diverse team of individuals with a range of expertise, knowledge and lived experience. They will work with you to identify the right mix of supports and make the best use of your funded supports in your plan. Support Coordination is a capacity building support that will assist you to understand your NDIS plan and engage the right NDIS providers and services based on your circumstances, needs and preferences.

Your Support Coordinator can also utilise your funding to assist you in finding suitable independent home and living supports.

At My Ability Pathway (MAP), our Support Coordination service is dedicated to helping you develop a personalised Support Map, which serves as a roadmap for coordinating and delivering the support you need. Your Support Map will outline the connections and providers necessary to help you achieve your goals, ensuring that every aspect of your plan is well-coordinated.

Support coordination begins with building your capacity to connect with informal, community, and funded supports, allowing you to make the most of your NDIS plan and pursue your goals effectively. As you progress, our support coordinators work closely with you to develop the skills to understand and use your plan. They ensure a balanced mix of supports is in place, enhancing your ability to maintain relationships, manage service delivery, live more independently, and be fully included in your community.

With your tailored Support Map, we ensure you have a clear and actionable guide to navigate your NDIS journey and reach your desired outcomes.

Specialist Support Coordination

MAP offers Specialist Support Coordination to people with more complex needs. The focus is on reducing complexity in the support environment and helping to overcome immediate and significant barriers. Our specialist support coordinators identify and understand the obstacles impacting an individual’s ability to access, use and maintain their support, leading them to find and try solutions to address these barriers.

At My Ability Pathway we are committed to ensuring that everyone has the opportunity to live a fulfilled and enriched life in a home that is not only comfortable but also suited to their unique needs. We recognise that for people with disability, achieving independence often requires specialised equipment and advanced technology. Our mission is to provide the necessary support to help you attain the level of independence you choose and deserve.

To achieve this, we carefully develop a personalised roster of support tailored to your inpidual goals and requirements under the NDIS. A comprehensive support plan is designed to assist with all aspects of personal care, health and wellbeing, access to community, family and social engagement, and household management, including financial and domestic needs. Whether it’s budgeting, managing household expenses, connecting with family or maintaining a clean and organised living space, our team is here to help every step of the way.

Our SIL services provide 24/7 support, including active or passive overnight care, to ensure your needs are met at any time of day or night. The support extends beyond the home keeping you connected to family, friends and helping you access the community, whether it’s for essential activities like shopping, attending medical appointments, or participating in social engagements. We understand the importance of staying connected with family, friends, and the broader community, which is why we provide assistance with attending events, engaging in sports activities, and participating in social gatherings.

Our Supported Independent Living (SIL) houses and dedicated team members are always available to ensure that you live your best and most comfortable life. At My Ability Pathway, we believe that with the right support, everyone can achieve their goals and enjoy a fulfilling, independent life.
