My Ability Pathway (MAP) was founded with a commitment to building connections, resilience and skills that create strong positive relationships.

My Ability Pathway (MAP) will provide you the support you need, for the life you want in the real world. This could mean attending community activities, dealing with domestic life, going to school or work, taking a much-needed holiday or heading out socialising, our staff will be with you, for you.

Whether it is 3 hours or 24 hours, we have the flexible support options to keep you living the life you choose.

Assistance with Daily Life

MAP provides a range of services to participant’s in their homes and the community, these range from personal care, domestic assistance, attending therapeutic sessions and other appointments, engaging socially and community activities.  We strive to create a solid match with our support workers to ensure participants receive quality and consistency in the services they receive. Note – a minimum of 3 hours per shift.

Social and Community

MAP emboldens participants to engage in their community and enjoy a life that is fulfilled and inclusive of social engagement. Participants choose how they want to engage and interact in the real world and MAP finds the right people to support them on that journey. Note – a minimum of 3 hours per shift.

Short Term Accommodation

MAP can offer a range of options for short term accommodation where a participant may need a change of environment, for them and their informal supports. MAP will work with the participant and their informal supports to find the right match of 1:1 support and location.

Medium Term Accommodation

MAP will work closely with participants to secure Medium Term Accommodation that will enable them to live safely and securely whilst they await the transition to their long-term housing solution.  A range of options will be explored and the best match for housing and supports will be developed around the participant’s needs.

Supported Independent Living

MAP provides participants with the support they need to have the real-life they want 24 hours a day 7 days a week.  MAP can join participants on the house hunt, be there when they move in, and support them in running their home, their way.

Specialist Support Coordination

MAP specialises in reducing barriers to managing the challenges that can occur in your support environment due to higher or more complex needs. Our Specialist Support Coordinator has a wealth of experience, knowledge and qualifications to help you implement your NDIS Plan.
